Betsi Cadwaladr: Health board to be prosecuted regarding mental health ward death

BCUHB is accused of breaching Section 3(1) of the Health and Safety at Work Act.

After news of the prosecution surfaced in April, Dr Nick Lyons, interim deputy chief executive and executive medical director from at BCUHB, apologised on behalf of the board for its “failures” in a “very tragic case”.

The health board was placed back into special measures on February 27.

Welsh Government health minister Eluned Morgan said at the time that this was due to “serious concerns about performance, leadership and culture”.

The chair, vice chair and independent members of the board were removed, with new independent members appointed in their place.
